Application Instruction

  • To request that an application packet be mailed to you, please e-mail with your complete mailing address information (name, address and phone number).
  • Complete application and return application materials to our Office of Admissions by postal mail.
  • Be sure to include payment of the $50 USD orientation fee which must be paid by check (payable to “SUNY Empire State College”), or paid by credit card.
  • Mail completed application form and payment to:
