| Federal Student Aid Applications |
| 1. Federal PIN: The first step in the application process for Federal student aid is to apply for a PIN. The PIN serves as the student's signature on the FAFSA and online MPN. A parent signature will be required only when parental data is changed on the application. You only need to apply for a PIN once in your college career. If you need a PIN, have forgotten your PIN or think someone else knows your PIN, you can request a new one at http://www.pin.ed.gov. |
| 2. Free Application for Federal Student Aid (FAFSA): Link to the Federal Department of Education's online application (Empire State College federal code = 010286) and complete the 2009-2010 FAFSA. If you desire a student loan, you must indicate that you want a loan (question #31) on the FAFSA. The Federal processor will not accept 2009-2010 FAFSA applications prior to January 1, 2009. |
| 3. Free Application for Federal Student Aid (FAFSA): Make online corrections to your FAFSA. Corrections on the Web allows students to make changes to all information except their Social Security number and date of birth. Students who wish to make changes to their application using Corrections on the Web must have a PIN in order to access their record. This PIN also serves as the student's signature. A parent signature will be required only when parental data is changed on the application. |
4. Entrance Loan Counseling: Link to online student entrance loan counseling for Direct Stafford loans and Direct PLUS loans. Required for all first time loan applicants at Empire State College.

5. Master Promissory Note: Link to an electronic Master Promissory Note (MPN), which is the student loan application. All loan borrowers for 2009-2010 will be expected to complete a new MPN in order to transition to the Direct Lending Program unless the borrower has had a Federal Direct Loan at another institution within the past 10 years.

| 6. Exit Loan Counseling: Link to online student exit loan counseling for Direct Stafford loan and Direct PLUS loan borrowers. Required for any loan recipient who will no longer borrow student loans with Empire State College. |
| 7. TEACH Grant: Learn more and access the required TEACH Grant forms (Only available to Graduate students.) |
| 8. Federal Perkins Master Promissory Note: Only complete this if you have been instructed to do so by the Financial Aid Office and Federal Perkins has already been awarded to your account. |
| 9. Verification Form: Complete the form online and electronically submit it to the Financial Aid Office along with a signed copy of your (and your spouse's if married; parents' if dependent) 2008 federal income tax return and W-2 Wage statements. (Complete the 2009-2010 Verification Form only if you are instructed to do so by the Financial Aid Office.) |
| 10. Negligible Income Form: Complete this form if requested to document how you provided for rent, food, clothing, etc for the period of 1/01/08-12/31/08. |
| 11. Special Condition Form: Find out about circumstances that allow the financial aid office to adjust your Federal Pell Grant. Link to a Special Condition Form. Complete the form online and electronically submit it to the Financial Aid Office. |
| 12. Dependency Review Form: Link to information and print an application for review of your federal dependency status. |
| 13. In-school Deferment Request Form: Link to an In-school Deferment Request Form so that you can defer student loan payments for prior federal student loans while you are enrolled at Empire State College. |
| 14. Permission to Pay Back Balance: Link to information and print the authorization form to use your credit balance from financial aid to pay a back balance on your account. Note: The credit applied cannot exceed $200 if the balance is from a previous academic year. |
| 15. Permission to Release Information: Link to information and print the authorization form to allow others to discuss your student records or financial aid and billing information with the Office of Student Financial Services. Students need only complete one form for all types of aid unless the form is rescinded. |
| 16. Drug Clearance: Download and complete this 2009-2010 worksheet to determine if you are eligible for aid after reporting on the FAFSA that you had a conviction for possessing or selling illegal drugs or after leaving question 23 of the FAFSA blank. This completed worksheet can be faxed to the Financial Aid Office at 518 581-2782. |
| 17. VA Education Benefit Received Form 2009-2010: This information is no longer required due to Bill H.R. 1777 which excludes VA benefits from being considered when packaging financial aid. Please contact us if you receive a request from our office for this information at 1 800 847-3000 or financialaid@esc.edu so that we may waive this request for you. |
| New York State Aid Applications |
1. Funding for this year has been exhausted. New York State Aid For Part-time Study: A grant program for students attending school for less than 12 credits per term. APTS is an allocated grant awarded on a first come, first serve basis.
If you and/or your spouse (and parents if dependent) filed a 2008 state income tax return you MUST submit a copy of the taxes with this application.

| 2. Express TAP Application (ETA): Students can complete the ETA online at the end of the FAFSA form. Students who did not complete the online ETA with the FAFSA may complete the ETA directly on the HESC web site. Students who meet certain demographic criteria will not need to complete the ETA. The corporation will notify you if you are not required to submit the ETA. The TAP School Code is 0913 for undergraduates and 5680 for graduate students. |
