Identity and Statement of Educational Purpose

Students may be selected to verify their identity and sign a statement of educational purpose. If chosen to complete this process, financial aid will not be determined at Empire State University until this requirement has been satisfied. If requested, please download the appropriate year form below.

UPDATE: Due to COVID-19 and the continued closure and/or limited access to university locations, students may submit the completed notarized form (form B) below instead of visiting a university location (form A) in-person to complete the requirement.  Students must include a copy of their identity documentation.
