Transcript Request and Form

We do not have the capability to email grades or send digital transcripts.

An official transcript, bearing the seal of the college and the signature of the Registrar, is a document required by colleges, universities and prospective employers. An official transcript can only be sent when a signed request has been received from the student.

Transcript requests can only be submitted in writing and must contain the original handwritten signature of the student. Transcripts cannot be released without the signed consent of the student. Any incomplete requests or those that cannot be processed due to outstanding financial obligations with the college will not be processed.

To request a transcript:

  1. Complete the PDF form and sign it.
  2. Send the completed form to the office of the registrar. (See mail and fax information below. By federal law, email requests cannot be considered consent for release of transcript information.)
  3. Send payment to the office of the registrar. (The cost of each transcript is $5. If you are requesting your first transcript ever, then this one transcript will be free of charge.)

Order Information

Transcripts are processed in the order they are received. Transcripts are usually mailed within two weeks of the receipt of your transcript request. During periods of heavy volume (such as the start and end of the semester) please allow additional time to process requests. We make every effort to meet specified deadlines. Please submit your transcript requests a minimum of two weeks prior to the date needed.

There may be a delay if there are outstanding academic documents and follow up is needed to complete your record. In this case, a notice will be sent to you at that time.

Mail Options

Transcripts will be sent immediately unless otherwise indicated on the request form. You can select to have your transcript mailed:

  • After degree/cerificate is posted
  • After final grades are processed
  • After degree program is concurred

Mailing Address

To ensure proper delivery, be make sure to provide a complete mailing address. We will mail the transcript to the address provided.
