Register with our Office and Request Accommodations

You, the student, drive the process of requesting accommodations for a disability by determining when, and if, you are going to disclose your disability to the university. Accommodations are not provided until you officially disclose your disability by submitting the Disability Declaration and Request for Accommodations. Accommodations received from previous institutions (i.e., former high schools and colleges) do not automatically carry over to SUNY Empire.

Here are the first steps to officially disclose and request accommodations:

  1. Complete the Disability Declaration and Request for Accommodations form. Determining the functional limitations of your disability may assist you in determining what accommodations you would like to request.
  2. Submit your completed form to the Office of Accessibility Resources and Services.

All students registered with the Office of Accessibility Resources and Services are responsible for knowing the information provided in the Office of Accessibility Resources and Services Student Handbook.
