In-person COVID Vaccine Requirements

SUNY requires all students taking courses in person or using services onsite to verify they are vaccinated. They may also wish to verify they’re vaccinated in case they wish to come onsite in the future. Here’s how:

  • Go to SUNY Vaccination Verification Application.
  • Log in using your SUNY Empire username and password.
  • Fill out the information, including any other SUNY campus you attend. You do not need to select SUNY Empire because your login and password do that for you.
  • Click the blue “verify” button to submit. You must consent and authorize SUNY to access information about your vaccination status at the top of the form.
