New Electronic Form for Undergrad Students in Education & Accounting

Students who are required to submit Teacher Certification or Accounting disclaimer forms now have an electronic option.  

Undergraduate students seeking a degree in the Area of Study of Education and those with goals to become NYS certified teachers are required to complete and submit a Teacher Certification Disclaimer form. Similarly, undergraduate students majoring in Accounting are required to submit an Accounting Disclaimer form. These forms must be completed prior to degree program concurrence (approval).  

Previously, paper forms with original signatures were required. Now, electronic versions of these forms can be authenticated utilizing login credentials. Both forms are accessed from the Registrar’s Office forms page. Students will need to log in using their SUNY Empire Credentials and agree to the attestation.  

Once the electronic form has been submitted, a note verifying its receipt will be visible in DP Planner, under the “total credits” calculation. 

Concurred students who previously completed and submitted one of the pdf/paper versions of the forms are not required to also complete the electronic attestation. For those students, the completed disclaimer form will remain as part of the student’s degree program portfolio.
